(1). Curriculum Vitae (Autobiography, Catalog of published articles from within the last five years, etc.)
(2). Abstract of highest degree’s thesis or portfolio
(3). Photocopies of Ph.D. diploma and M.A. diploma
(4). Photocopies of teacher’s certificate; applicants who do not have the teacher’s certificate please submit two copies of the highest degree’s thesis.
(5). Official transcripts of graduate studies
(6). A Photocopy of ID or Valid ARC/APRC
(7). other favorable documents for review
